A factual account of what the public record shows about an operator’s safety history — produced at no cost, for any organisation that wants one.
IRVINE, CA — September 1, 2026 — EON AI Ventures today launched the Pattern Report, a free service that compiles what the public record already shows about safety incidents at an industrial organisation, and identifies where those events repeat.
The information is not new. Regulator enforcement records, official investigation reports, company disclosures and industry statistics are all public. What is rarely done is the work of assembling them for a single organisation and asking a simple question: has this happened here before, in this same kind of task?
That question is the report. Where an operator has had three events involving the same task family — isolation, confined space entry, hot work — the report states the recurrence and shows the same pattern documented at other operators. It draws no conclusions about the cause and names no individuals. The methodology, evidence framework, and approach to identifying industrial safety patterns are detailed in the accompanying white paper, “The Record Was Always There.”

How it works
Every claim is traced to a public source and listed in full. An incident is included only where two independent public sources support it, or where the source is a regulator, an official investigation body, or the organisation’s own disclosure. Items that cannot meet that standard are dropped rather than qualified. Every report is reviewed by a named person before release, and any organisation that shows an item to be wrong has it corrected and the report reissued, with the correction logged publicly.

What it does not contain
The Pattern Report contains no information from any EON customer, no assessment of an organisation’s competence, no attribution of blame where an investigation has not concluded, no named individuals, and no EON product content of any kind. It is free permanently, with no gated version and no paid tier.
Why EON is doing this
In upstream oil and gas in 2024 there were 32 fatalities across 21 incidents, and 26 of those were contractors — people who did not write the procedure and had the least standing to stop the job. The patterns behind those events recur across companies and are usually rediscovered independently, often after another incident.
EON’s published data rights standard obliges the company and its customers to permit disclosure of any pattern bearing on life safety and forbids either party from suppressing such a finding. The Pattern Report applies the same principle to information that is already public.
